An illustrative example.

Say a week of care is billed at $1,000, and your plan has a $2,000 out-of-network deductible and then reimburses 60 percent of what it considers an allowed amount.

Until that $2,000 deductible is met, you pay in full and the superbills you submit go toward meeting it. After it is met, if the insurer decides the allowed amount for those services is $800, it would reimburse 60 percent of $800, which is $480, and the remainder stays with you.
