Leaving a residential program is one of the most fragile moments in the recovery process. The structure, safety, and individualized attention that made residential work can disappear overnight when someone returns home. Continuing Care at Heights Behavioral Health is built to close that gap. We are designed to preserve the individualized experience clients receive in premier residential programs while helping them reintegrate into everyday life in Houston.

This is continuing care that is elevated by design. It is built to feel less like a standard outpatient program and more like a private practice that already knows your history, customized around the person rather than a fixed schedule. After a comprehensive assessment guided by ASAM criteria, we recommend the right level of care, then adjust it as life at home stabilizes.

What Continuing Care Includes

Every plan is individualized after a thorough assessment. Depending on clinical need, continuing care can include Individualized Intensive Programming, PHP, IOP, or evening IOP, paired with individual therapy, group work, and family involvement. For the parts of life that happen outside the therapy hour, we add non-clinical mentoring and companioning through Heights Mentoring, so the transition home has real-world support alongside clinical care.

The goal is continuity. The same level of individualized attention, the same clinical depth, carried from residential into a plan that fits a person’s actual life. We use ASAM criteria to keep the level of care honest, stepping support down as someone stabilizes and back up if they need more.

Coordinating With Your Residential Program

We work directly with residential teams, discharge planners, and families to make the handoff seamless. With the client’s consent, we coordinate the clinical transition so nothing is lost between programs and the plan continues without a gap. We collaborate with residential partners rather than endorse them, and we keep every plan centered on the individual.

What is continuing care after residential treatment?

Continuing care is structured outpatient support for adults who have completed a residential or inpatient program. It preserves the individualized clinical attention of residential treatment and helps the person reintegrate into work, family, and daily life at home, with the level of care adjusted as they stabilize.

How do you decide the right level of care?

We begin with a comprehensive assessment guided by ASAM criteria, then recommend a level of care that fits the clinical picture and the person’s life. We step support down as someone stabilizes and back up if they need more.

Do you coordinate with our residential program or interventionist?

Yes. With the client’s consent, we work directly with residential teams, discharge planners, interventionists, and families to coordinate a seamless clinical handoff and a continuing care plan built around the individual.

Do you take insurance for continuing care?

We are private pay and also work with out-of-network benefits. We will explain costs and verify your specific benefits before you commit to anything.

What is continuing care or aftercare?

It is the ongoing, lighter support that follows a more intensive level of care, designed to protect your progress, keep you connected, and catch challenges early as you return to daily life.

Can I start continuing care if I did treatment somewhere else?

Yes. We welcome people stepping down from residential or another program. We will review where you are and build a continuing care plan that picks up your momentum.

How long does continuing care last?

As long as it is helpful. Many people taper gradually over months. Your plan is customized and adjusts as your confidence and stability grow.

Is continuing care available virtually?

Yes. Much of continuing care, including groups and individual sessions, can be done by telehealth, making it easier to stay consistent long term.

I just finished a 30- or 60-day residential program. Is Heights the right next step?

Yes. This is exactly who we are built for. The weeks right after residential are when progress is most fragile, and a structured next step is what protects it. We pick up where your program left off and continue the work at home, at your pace.

Do you coordinate with my residential or detox program?

Whenever possible, yes. With your permission we connect with your prior treatment team so your history, goals, and momentum carry forward, and nothing important gets lost in the handoff.

What if I need more structure than weekly therapy after residential?

We offer a full range of step-down intensity, from our Individualized Intensive Programming and PHP/IOP through to lighter outpatient and continuing care, so you can step down gradually rather than going from full structure to nothing.
